All patients using this protocol receive a home exercise kit to support the training/exercises they are taught in their physical therapy treatment sessions. Since patients are usually seen once a week and for a minimum of three months, skill development is not possible without the support of the educational material and devices in the kit.
For Worker’s Compensation cases, the physician orders the kit as part of the treatment request, and the therapist orders the kit from Medical Dynamics, Inc., an equipment supplier in Northern California.

To have the order billed to the insurance provider, therapists should download
this PDF file and fax your order to Medical Dynamics at the number on the
form. Enclose the physician’s order and Medical Dynamics will get authorization
from the insurance carrier and ship directly to the patient.
